Many older adults miss life-saving influenza vaccines each winter, especially in low- and middle-income countries. We conducted a factorial randomized controlled trial testing three interventions: (i) a U.S.-style reminder nudge, (ii) one-off subsidies ranging from $1.50 to $15, and (iii) cost information introduced after re-randomization. The reminder nudge and cost information had no impact on vaccination. One-off subsidies substantially increased both vaccination intentions and behaviors, with small incentives (from $3 upward) more than doubling uptake. Subsidies also raised re-vaccination in the following year. Returns to vaccination were heterogeneous, with greater medical cost savings among individuals with lower unobserved resistance. On average, influenza vaccination saved $431 in medical expenses, yielding a benefit–cost ratio of 40.

每年冬季,许多老年人未能接种可挽救生命的流感疫苗,这一问题在中低收入国家尤为突出。我们开展了一项析因随机对照试验,检验三种干预措施的效果:(1)美式提醒型“助推”,(2)一次性补贴(金额介于1.50美元至15美元之间),以及(3)在重新随机分组后提供疫苗费用信息。结果显示,提醒型助推和费用信息均未显著影响接种行为;一次性补贴则显著提高了接种意愿和实际接种率,其中小额激励(3美元及以上)即可使接种率增加一倍以上。补贴还促进了次年再次接种。接种收益存在异质性,潜在抵触程度较低的个体获得了更大的医疗费用节省。平均而言,流感疫苗接种可节省431美元的医疗支出,其收益成本比为40。
